Late ADHD diagnosis has become common, and the reaction to it is usually a mix of relief and grief — relief at an explanation, grief for the twenty years spent believing you were lazy.

Why It Was Missed

The classic picture is a disruptive boy who cannot sit still. That picture is a screening instrument, and it filters out an enormous number of people.

It was missed if you were bright. Intelligence compensates for a great deal, and a child who is struggling but still scoring well does not get referred.

It was missed if you were a girl. ADHD in girls more often presents as inattentiveness and daydreaming than as disruption, and quiet children are not sent for assessment.

It was missed if you were anxious, because the anxiety got treated and the thing underneath it did not.

And in an Indian schooling context specifically, it was missed because inattention and disorganisation are read as character defects to be corrected with discipline rather than as anything requiring assessment. A great many adults were told for years that they simply were not trying.

What It Looks Like at Thirty-Five

Not hyperactivity. By adulthood that has usually turned inward — a restlessness rather than a physical inability to sit.

  • Losing hours to something absorbing, and being unable to start something dull no matter the consequences
  • Time that does not behave: chronic lateness, no reliable sense of how long anything takes
  • A functioning life held together by elaborate scaffolding — alarms, lists, backups for the backups
  • Starting enthusiastically and finishing rarely
  • Emotional intensity that arrives fast and passes fast
  • Rejection sensitivity, where mild criticism lands as devastation
  • Exhaustion, because performing organisation without an internal organiser is enormously effortful

The paradox that confuses people most: the inability to focus and the ability to focus for nine unbroken hours are the same condition. ADHD is dysregulated attention, not absent attention.

Diagnosis, and Being Straight About Who Does What

ADHD is diagnosed by a psychiatrist or an appropriately qualified clinical psychologist, through structured assessment and developmental history. It is not diagnosed by a counsellor and it is not diagnosed from an online quiz.

Medication, where it is appropriate, is prescribed by a psychiatrist. It is well evidenced and helps a substantial majority of people.

What a counsellor can do is different and complementary: work out whether assessment is worth pursuing, help you prepare for it, and deal with everything the diagnosis does not touch.

What Counselling Actually Helps With

The grief. Almost nobody expects this part. A diagnosis at thirty-eight reframes school, a career you chose badly, and relationships that ended for reasons you now understand differently. That is a real loss and it deserves attention rather than being skipped over on the way to practical strategies.

The self-concept. Two decades of being told you were careless does not dissolve because a doctor gave it a name. Most people carry a deeply held belief that they are fundamentally unreliable, and that outlasts the diagnosis by years.

The relationships. Partners of undiagnosed adults have frequently spent years feeling like a parent, and the resentment on both sides is substantial. This work is often best done together.

Practical systems built for how your attention actually works, rather than the systems that have failed you repeatedly because they were designed for someone else's brain.

Two Honest Cautions

Not everything is ADHD. Anxiety, depression, chronic sleep deprivation and thyroid problems all produce poor concentration, and a proper assessment exists to distinguish them. Self-diagnosis from social media has a real false-positive rate.

And a diagnosis is not a personality. It explains a mechanism; it does not account for everything you have ever done, and it should not become the whole story.

Can you diagnose ADHD?

Is it worth getting assessed as an adult?

For most people, yes — because it changes what you can access and, just as importantly, how you interpret your own history. Medication and structured support both need the diagnosis first.

Does medication mean I do not need therapy?

Medication typically improves attention and impulse control. It does not repair self-esteem built over twenty years, a strained marriage, or the absence of workable systems. The two do different jobs.

Could it be anxiety instead?

It could be anxiety, and the two overlap heavily and often co-occur. Proper assessment separates them, which is one of the better arguments for having it done rather than self-diagnosing.
